The doctors welcome new patients who live within our practice catchment area.
If you are currently registered with another practice within the Redditch area, we advise you remain with your current practice until such a time that you are asked to find an alternative practice; for reasons such as moving out of your practice catchment area.
As it often takes some time for records to be forwarded from your former practice, all newly registered patients will be asked to complete a health questionnaire. Medical treatment is available from the date of registration, please contact reception for further information.
You may print off a registration pack, complete it and bring it in with two forms of ID. Details of accepted documents are noted in our registration pack, but must include one form of photo identification and one of proof of address dated within the last three months.
For children aged 5 years of age and under, we ask you to present your child's red book health record at the time of registering. Please note we cannot register a new baby without their NHS number.
Sharing Your Summary Care Record (SCR)
The NHS Summary Care Record (SCR) is an electronic summary of key clinical information (including medicines, allergies and adverse reactions) about a patient, sourced from the GP record.
It is used by authorised healthcare professionals, with the patient's consent to support their care and treatment. If you haven't already consented to opt in to share your SCR, please download and complete our consent form and return to the surgery reception.
If you wish to Opt out of the SCR please enquire at reception and a member of our team can provide with the opting out application.
For anyone 16 years of age or older that requires another adult to represent them or discuss their medical matters. Including management of prescriptions, correspondence with the surgery or to book appointments on their behalf they can chose a patient representative. In order for us to facilitate this we require patient consent, in accordance with Information Governance and the GDPR.
A patient consent form can be found below this can be downloaded, completed and returned to the surgery reception.
When returning you're consent form,you must be in attendance and a member of our surgery staff must witness the you signing the form.
If a patient lacks the capacity to sign or is unable to attend surgery, then the patients representative must attend with both their own and the you're photo identification in order for the surgery to confirm implied consent.

Guide to GP Services
The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them. You can download the guide by clicking the link below.
If you become unwell whilst away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local a GP practice for up to 15 days. This will allow you to be seen at a local practice and still remain a permanent patient of your registered GP surgery.
After 15 days your your temporary residency will expire and we will return any records that we hold to your regular GP. If you still require treatment from us you will need to register as permanent patient.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient, although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
Please be aware that due to both legal and contractual requirements all temporary residents must present to the practice with photo identification. If you don't not have any photo identification we suggest you attend an alternative service such as out of hours GP, Minor Injuries Department or a Accident and Emergency Department.
